Findings & Observations Grid

A Finding is a deficiency, nonconformity, or observation as the result of an audit, inspection, internal review, program manager action, etc. An Observation is any item that the auditor wants to document for the purposes of greater visibility and/or planned actions (such as making it a best management practice, incorporating it as a success story, etc.).

Findings and Observations can be added to the Findings & Observations grid via the “New Finding / Observation” button.

The Query icon and its dropdown menu are found to the right and directly above all major grids. The Query icon opens the query form where queries of the data in the grid beneath are performed.

If you wish to save a query for use in the future, immediately click on the alt text Active Query icon and a "Save As" link will appear on the top right of the Query form. Click on "Save As", name the query and click "Save". The name of the query will now appear under the Query icon dropdown. To activate the saved query, click on the triangle to the right of the icon and select the query. This is a friendly method to make frequently used queries easy to access with just a single click of the mouse.

The Print icon and its dropdown menu are found next to the Query icon directly above all major grids. The Print icon opens the print form where exports of the data in the grid beneath are performed.


Query Findings & Observations Grid Form

Description

The Query Findings & Observations Grid Form allows you to query/filter data in the grid by specific fields. Once you enter a word, part of a key word, date(s) of interest, or any combination of these, click on “Search” and the grid will filter to reveal your queried data.

Definitions

  1. Number: This field filters for the number of the Finding / Observation.
  2. Organization: This field filters for the name of the Organization where the Finding or Observation information was originally inputted into EMSWeb.
  3. Subject: This field filters for the Subject of the Finding / Observation.
  4. Media: This field filters for media that is related to the Finding / Observation.
  5. Type: This field filters for the Finding Type. Options include EMS, Compliance, or Class I-IV.
  6. Finding Category: This field filters for the Finding Category.
  7. POC: This field filters for the person responsible for managing the resolution of the Finding. If the Finding requires a formal review prior to closure, the POC can forward the Finding to the Auditor for closure. If the Finding does not require a formal review, the POC can resolve and close out the Finding.
  8. Status: This field filters for the status of the Finding / Observation [Draft, Open (Not Assigned), Open (Assigned), or Open (Resolved), Closed, Reopened (Not Properly Resolved), Pending Formal Review, or Completed Formal Review].
  9. Progress: This field filters for the percentage of progress towards the completion (100%) of the POAM of the Finding / Observation.
  10. Last Action___to___: This field filters for the date that the most recent action (e.g., Opened, Assigned, Resolved, Closed, Reopened) was recorded in the History section of the profile of the Finding / Observation.
