Description
This form is used to Add Checklist Templates or Edit Checklist Templates.
There are three types of Checklist Templates: General, Compliance Audit, and EMS. All three templates provide a comment field for each question, but they differ in the structure of the question and the types of answers.
- The General Template contains a simple text field for your question and a section header (you can divide questions into different sections within a checklist). Possible answers for each question are “Yes,” “No,” and “N/A.”
- The Compliance Audit Template is perhaps the most complex and was designed to accept imports from Listbuilder (Team Guide). Section headers, Item ID, Citation Code, Reference Details, and the text of the question are all fields used in the checklist. The Item ID appears to the left of each question on the checklist form, and the available answers are “Compliant,” “Noncompliant,” and “N/A.”
- The EMS Template is similar to the General Template except for the addition of an Item ID (typically the ISO 14001 element number) and the “Conform,” “Nonconform,” and “N/A” answers.
Checklist templates have headers and footers that can be customized, and any question created can be edited and organized as you see fit. Extensive alerts can be assigned based on the answers given per question or for the entire template upon submission of the completed checklist. For example, an alert can be assigned to the template with the following criteria: send me an email when any question is answered “No” or when the third question is answered “Yes.”
Definitions
- Checklist Template Name: Name of the Checklist Template
- Jurisdiction: The state (e.g., Maryland) that is associated with the Checklist Template.
- Template Type: Type of Checklist Template (Simple Checklist, Compliance Audit, or EMS Audit)
- Organization: Name of the Organization that is inputting this Checklist Template information
- Media: Specific areas of the environmental field typically defined according to environmental programs such as air, water, tanks -- which are the subject of regulatory concern and activities.
- Instructions: Instructions for the end user to follow when filing out the Checklist
- Share with other organizations: Place a check in this box to make the Checklist Template viewable by Users at this Organization AND by Users at any of its child Organizations.
- Set Inactive: Place a check in this box to remove this Checklist Template from any applicable dropdown menus.
- Restart numbering for each section: Place a check in this box to restart numbering in each individual section, or leave it blank for continuous numbering throughout the checklist.
- Date Scheme: Type of Date Scheme to be used in a Header Field for this Checklist Template (either None, Start & End Date, Date, or Date & Time)
- None: No Date Scheme Header Field
- Start & End Date: a Header Field that incorporates Start & End Date
- Date: a Header Field that incorporates Date only
- Date & Time: a Header Field that incorporates Date & Time
- Auditor/Inspector: Place a check in this box to include a Header Field for Auditor/Inspector information on the Checklist Template being created.
- Submission Date: Place a check in this box to include a Header Field for the Submission Date on the Checklist Template being created.
- Building: Place a check in this box to include a Header Field for Building information on the Checklist Template being created.
- Room: Place a check in this box to include a Header Field for Room information on the Checklist Template being created.
- Location: Place a check in this box to include a Header Field for Location information on the Checklist Template being created.
- POC Name: Place a check in this box to include a Header Field for the POC Name on the Checklist Template being created.
- Contact Info: Place a check in this box to include a Header Field for Contact Information on the Checklist Template being created.
- Evaluated Item: Place a check in this box to include a Header Field for the related EMS data that will be evaluated on the Checklist Template being created.
- Additional Comments: Place a check in this box to include a Footer Field for Additional Comments on the Checklist Template being created.
- Signature Block: Place a check in this box to include a Footer Field for a Signature Block on the Checklist Template being created.
