Description

Once created and saved, a Finding / Observation goes through an Assignment, a Resolution, and a Closure phase. This form is used to Edit the Resolution of the Finding / Observation.

2. Root Cause Code: The Navy standardized list of root cause codes is listed here in a drop down menu format. Choose the closest root cause code the reflects your situation. Root cause is defined as the cause of an occurrence that, if corrected, would prevent recurrence of that and similar occurrences. There may be a series of identifiable causes, one leading to another. Activities should pursue that series of causes until identifying the fundamental correctable cause.
3. Root Cause Description: Use the description field to more fully describe the root cause.
4. Corrective Action: action to eliminate the cause of a detected nonconformity
